If you currently hold a valid, or recently expired, 10-year passport, you may use the following guidance to apply for your new passport.
1. Fill out and print FORM DS-82
Complete FORM DS-82 online, and print it out single-sided. Back to back printed forms cannot be processed. Do not print the instructions as you are to use this website instead. At ‘Mailing Address’ list your Dutch address, even if it is only temporary. You cannot currently list a “Country” when completing the “Emergency Contact” section on our forms. Please list an emergency contact in the United States. You may sign the form at home. Applications filled out by hand may require longer to process.
2. Social Security Number requirement
You must provide an answer to question no. 5, even if you do not have a Social Security Number or do not remember it. Do NOT list your Dutch Burger Service Number (BSN). Click here to learn more about passports and the social security number requirement.
3. Proof of U.S. Citizenship
Present your most recently issued full validity (10-year) U.S. passport, with a photo copy of the biographical information page. If your most recently issued passport expired more than five years ago, click here.
4. One U.S. format color passport photo
Provide one U.S. format: 2″ x 2″ (5 x 5 cm.) passport photo. We cannot process the smaller Dutch format photos! Click here for guidance on the perfect U.S. passport photo. Make sure to watch the video.
5. Bring photo copies
Bring a plain photo copy of:
- Your passport’s biographical information page, and
- any supporting documents as applicable to your application.
Copies of the forms are not required.
6. Passport Application Fee
You can pay the appropriate passport services application fee in euro, in U.S. dollars, or by credit card. Dutch PIN-cards cannot be processed. Note that we can only process credit card payments if the card holder is available to sign the payment authorization slip. When paying in cash, please try to bring exact change. 7. Set up an appointment
Set up an appointment via the link to the appointment website elsewhere on this page. IMPORTANT: Print the appointment confirmation page or save it on your phone. You will NOT receive a confirmation e-mail.
Note that you can have someone else deliver and pay for your application, in person, by appointment. Make sure that:
– You have carefully prepared and signed your application, attached the correct format photo and provided your current passport, photo copy and supporting documents if any.
– The deliverer’s name is listed on your appointment and carries his/her passport or driver’s license as identification.
We cannot process paid passport applications by mail, you will have to set up an appointment.
ARRIVE ON TIME
Try to arrive at the Consulate General in Amsterdam, at least 15 minutes prior to your appointment time. If you miss your slot we must ask you to reschedule.
Note that slots open up if we receive cancellations. Check back with our appointment system regularly if you wish for a earlier appointment slot.
8. Name Change?
If your name has changed through marriage or divorce, follow the above guidance but also provide your original marriage certificate or divorce decree, with a plain photo copy of this document. Original documents will be returned to you.
9. New passport will be delivered by mail
Provide an A5 (C5) size envelope (approx. 23 x 26 centimeters), legibly addressed to the passport applicant, with a PostNL registered mail bar-coded sticker. Retain the tracking number. Note that you must allow us to cancel the current passport if it is still valid at the time of your appointment, as you cannot have two valid passports. Plan your appointment accordingly as you may be without a valid passport for up to three weeks.
If you are also applying for a passport card, bring two registered mail return envelopes as cards arrive two to three weeks after passport books.

Passports are printed in the United States with a secure, photo-digitized image of the bearer. You should allow for up to 15 working days for your application to be processed. If you are traveling within 15 working days and can provide evidence of this travel, you may be eligible for an emergency limited-validity passport. Please see our section on Imminent Departure.
